Definition of the Term Influential The term influential information, when used in the OMB Guideline in the phrase "influential scientific, financial, or statistical information, " applies when the agency can "reasonably determine that dissemination of the information will have or does have a clear and substantial impact on important public policies or important private sector decisions" 67 FR 8452; February 22, 2002 ; . However, because each agency is different, and there are vast differences in the types of information they disseminate, each agency has been asked to elaborate on the definition of influential in the context of their missions and duties, "with due consideration of the nature of the information they disseminate." As stated in the OMB Guideline V.9 ; , "[e] ach agency is authorized to define 'influential' in ways appropriate for it given the nature and multiplicity of issues for which the agency is responsible." For purposes of this guidance, influential information is defined as disseminated information that results from or is used in support of agency actions that are expected to have an annual effect on the economy of $100 million or more or will adversely affect in a material way the economy, a sector of the economy, productivity, competition, jobs, the environment, public health or safety, or State, local or tribal governments or communities. It should be noted that the definition applies to "information" itself, not to decisions that the information may support. Even if a decision or action by FDA is itself very important, a particular piece of information supporting it may or may not be "influential." Two examples of what FDA considers to be influential information follow.

By 10 days after infection, the untreated wildtype mice and NOS2-deficient mice no longer had detectable parasites, whereas all the L-NAME-treated mice still had measurable parasite loads. This difference between NOS2-deficient mice and L-NAME-treated mice could represent compensatory mechanisms at work in the genetically deficient mice or the inhibition of additional isoforms of NOS, e.g., the neuronal isoform NOS1 or the endothelial isoform NOS3, by L-NAME. To determine whether NOS-deficient or L-NAME-treated mice had markedly altered immune responses, we analyzed the production of anti-parasite IgA.

Approximately 45% of the initial bupropion hydroxylation activity and 42% of the CPA activity remained after incubation with 20 M efavirenz. These values were similar to the activity remaining when 7-EFC was used as the substrate Fig. 1 and data not shown ; . As seen with 7-EFC, preincubation of the variant enzyme with efavirenz did not result in inactivation as measured by the hydroxylation of bupropion or CPA data not shown ; . Reversibility of Efavirenz-Mediated Inactivation of P450 2B6. As shown in Table 3, the inactivation of P450 2B6 by efavirenz was reversible after overnight dialysis, and the enzymatic activity, reduced CO spectra, and heme remaining were completely restored. Topiramate has not been studied in double-blind trials for efficacy in prophylaxis or rapid cycling in bipolar disorder. However, trials of topiramate for other health concerns e.g., binge-eating disorder associated with obesity, 20, 59 alcohol dependence, 21 and migraines60 ; have had positive results. Zonisamide Although no studies of the effects of zonisamide in bipolar depression or prophylaxis have been conducted, results of a case series50 have shown some promise of efficacy in mania in bipolar patients. Fifteen of 21 patients with DSM-III-R bipolar and schizoaffective mania showed moderate-to-marked improvement on the CGI with adjunctive zonisamide 100600 mg day ; . These favorable results indicate a need for controlled studies of zonisamide for the treatment of mania. SUMMARY AND RECOMMENDATIONS The data reviewed in this article suggest that anticonvulsants do not have efficacy as a "class effect" in the treatment of bipolar disorder. In other words, only some anticonvulsants have efficacy in treating core bipolar disorder or symptoms while others do not. Gabapentin and tiagabine do not appear to have efficacy in treating acute mania while pregabalin has not been tested for its efficacy to date. Among those that have efficacy in treating bipolar disorder, the spectrum of efficacy seems to be different as well. For example, valproate and carbamazepine clearly have efficacy in treating acute mania while their efficacy in treating acute bipolar depression remains uncertain. Levetiracetam, oxcarbazepine, phenytoin, and zonisamide show promise in treating mania, but further studies are needed to confirm these observations. In contrast, lamotrigine clearly has efficacy in preventing bipolar depressive episodes and in treating acute bipolar depression, but it does not appear to have efficacy in treating acute mania. Case Report L.G. is a 72-year-old man with a past medical history significant for hypothyroidism and diminished hearing subsequent to a right mastoidectomy as a boy. Personal and family history for either neurologic or psychiatric diseases was denied, and the patient had never abused alcohol or illicit drugs. About 20 months prior to his current presentation he experienced an episode of vertigo, and the clinical examination by a neurologist, MRI of the brain, carotid duplex studies, and an EEG were all normal. A few months after that episode of vertigo, the patient's wife reported a change in her husband's personality marked by extreme anxiety and agitation, which appeared to have developed after an exterminator had been called to the home to place some mousetraps. For instance, milk chocolate candy typically contains 7 ingredients: 1 ; cacao paste; 2 ; cocoa butter; 3 ; sugar, usually sucrose; 4 ; nonfat milk solids; 5 ; milk fat; 6 ; an emulsifier, usually lecithin; and 7 ; flavoring, usually vanilla. Accordingly, any of the remaining ingredients in chocolate candy either individually or in combination with one another cannot be ruled out in the etiology of acne. As has been previously pointed out, 14 both the treatment and control bars, because of their similar high sucrose contents, would have each elicited high glycemic and insulinemic responses as a result of their inherent high glycemic.
TREATMENT OF DEPRESSION IN PARKINSON'S DISEASE and another 40% had anxiety symptoms. Furthermore, depression is highly comorbid with anxiety in these patients. Of patients with PD who have a diagnosis of depression, up to 67% also have an anxiety disorder.7 While it is not clear what impact anxiety has on longterm issues in PD, it has been associated with poorer long-term outcome in depressed non-PD populations.8 Furthermore, anxiety that is comorbid with depression in non-PD patients is effectively treated with antidepressants, which may improve long-term outcome.9 Despite the importance of depression, anxiety, and disability in PD, there are virtually no empirical data from well-designed controlled studies that can direct treatment. There are a number of older controlled trials of depression with tricyclic agents, but each of these is so methodologically compromised as to render the results nongeneralizable.1012 There is also a small controlled study of bupropion, 13 but it examined only 12 depressed patients, of whom 5 improved. Therefore, it is obvious that these studies are very limited in the extent to which they can inform clinical practice. While there are open label trials supporting the use of sertraline, 14, 15 paroxetine, 1618 fluoxetine, 18 and fluvoxamine18 for the treatment of depression in PD, none of these trials assessed the important outcomes of anxiety, disability, and cognition. There have been two open-label studies of citalopram in depressed PD patients. A study conducted by Rampello et al.19 had 18 patients, 15 with dysthymia and three with major depression, of whom 15 improved, though no details on the amount of improvement are given. Additionally, this study does not assess other important outcomes such as disability, anxiety, and cognition. In a study by Dell'Agnello et al., 18 62 patients were given fluoxetine, fluvoxamine, sertraline, or citalopram 15 patients ; . Dell'Agnello et al.18 reported that a significant improvement in depressive symptoms from baseline to the end of the study was achieved with all of the selective serotonin reuptake inhibitors SSRIs ; , without any difference between the four drugs. They did not, however, provide details of the degree of improvement on the depression measure, nor did they measure the outcomes of anxiety, cognition, and disability. There is one controlled trial of an SSRI, citalopram, 20 which found no difference between active drug and placebo, but there are significant concerns about the methodology of this study. A subtherapeutic dose of citalopram was used, depression was rated only at 0 and 6 weeks, and the authors seemed to indicate that many of the patients they entered into the study did not, in retrospect, have major depression. There also was no attempt to measure the outcomes of disability, anxiety, and cognition. As there are no treatment trials in patients with PD that assess the impact of treating depression on anxiety, disability, and cognition, an attempt to provide some preliminary data on this issue is important. We therefore undertook a prospective, open-label trial of citalopram in carefully diagnosed patients with PD to include an examination of the impact of treatment of depression on anxiety, disability, and cognition. We chose to examine citalopram, the most potent of the SSRIs, as it is now widely used as an antidepressant and it does not have anticholinergic properties or significant P450 interactions with other medications used in PD and other medical disorders.21.
