Three cancers continued to show substantial increases in mortality through 1995 in men and women. Liver cancer showed the greatest increase in mortality and incidence among both men and women, increasing 12.3% in men and 12.9% in women during the 5-year period Tables 2 and 3 ; . The increase in liver cancer rates are likely to be due to increased infection rates with the hepatitis C virus HCV ; and hepatitis B virus HBV ; , known hepatic carcinogens.6 Assuming this is the correct explanation, the long latency period between infection and cancer development indicates that mortality rates for this disease will continue to increase in the near future unless effective treatments are found. Mortality has also increased for multiple myeloma 5.6% in men and 3.8% in women ; , although it seems that incidence is declining. Mortality from NHL increased in both men and women by approximately 6%, and its incidence increased approximately 3%. The reason for the increases in rates for NHL during the past 20 years is unclear, although it seems that changes in diagnostic and classification practices may have contributed. In the sections below, we review the current state of knowledge and characterize the major risk factors that seem to be driving secular trends in cancer mortality and incidence. We categorize these sites by common risk factors, including smoking, diet, hormones, and other factors unique to a specific cancer site. Among these categories, we explore the potential impact of primary prevention, early detection, and treatment advances on future mortality and incidence patterns. Furthermore, we discuss what we have learned from past efforts in prevention and treatment and discuss promising directions for etiologic and prevention research.
| Is more controversial.82 Because consensus has not been achieved on these competing points of view, we chose to analyze the MCI outcomes alone and combined with probable dementia. When viewed independently from probable dementia, the study groups showed no statistical differences in the risk of developing MCI. The risk of developing either MCI or probable dementia increases by 37% for women taking estrogen plus progestin compared with women in the placebo group P .06 ; . One possible explanation for the lack of an effect of estrogen plus progestin on MCI alone may relate to the greater variability in cognitive status and greater heterogeneity in possible underlying diseases among the participants with MCI as opposed to the participants with probable dementia. Both of these factors limit the predictive power of MCI as well as its use as a classification in clinical practice. The ongoing follow-up of the full WHIMS cohort, including those participants identified as having MCI, and future studies in which consensus has been achieved on a more precise MCI designation, may help to clarify this point. Study drug administration in the WHI estrogen plus progestin trial was stopped on July 8, 2002, after an average exposure to the hormones of 5.6 years33; however, monitoring of important clinical including cognitive ; outcomes in these women continues in both the WHI and the WHIMS trials. Of particular interest in the WHIMS estrogen plus progestin cohort is the degree to which the negative effects of the hormone treatment have on dementia are sustained over time. The WHI estrogen-alone trial continues, as does the WHIMS estrogenalone component with its assessments of global cognitive functioning, MCI, and probable dementia. As with the estrogen plus progestin component of the WHIMS, the WHIMS estrogen-alone study is the largest of its kind with the same rigor in design and outcome ascertainment as the WHIMS estrogen plus progestin trial. Given the current findings, the results of the estrogen-alone component assume added significance and keftab, for example, cefpodoxime gonorrhea.
Physicians may be reluctant to prescribe an oral cephalosporin for a patient with a history of penicillin or other -lactam antimicrobial allergy out of concern for cross-reactivity and risk to the patient. However, most second- and third-generation oral cephalosporins cefpodoxime, cefdinir, cefixime, cefuroxime ; possess sufficient structural distinction from penicillin and amoxicillin in that they pose no increased risk of allergic reactions. They should, however, be used with caution in patients with a history of an immunoglobulin E IgE ; mediated adverse reaction.37 On the other hand, cross-reactivity to cefaclor, cephalexin, cefprozil, and cefadroxil appears to be common enough that these agents are best avoided.37 Type I IgE-mediated immediate hypersensitivity to penicillin is rare, with anaphylaxis occurring in 0.004% to 0.015% of drug administrations.37 In addition, patients with idiopathic or viral-related penicillin reactions do not have true IgE-mediated hypersensitivity and can be safely given a cephalosporin.37 Although we are concerned about the use of a parenteral -lactam agent in a patient allergic to penicillin, the AAP AAFP guidelines recommend parenteral ceftriaxone as an option for any patient with penicillin allergy who experiences severe otalgia, fever 39C, or gastrointestinal upset.18, 38.

This study explored the attitudes of GPs to NICE guidance and aimed to investigate any changes in prescribing patterns. Five technology appraisals most likely to impact on GP prescribing were investigated. PACT data were analysed for changes in prescribing patterns before and after the publication of each technology appraisal. A questionnaire was sent to all GPs within a single PCT to explore factors that were encouraging or discouraging adherence to NICE guidance. PACT data showed that there was an increase in the prescribing of the drugs studied immediately after NICE guidance, with the exception of zanamivir. Although there was an increase in the prescribing of maintenance doses of proton pump inhibitors, there was also an increase in treatment doses. In general, there was a balance between the factors that encouraged and those that discouraged adherence. The main exception was zanamivir, where factors that discouraged adherence greatly exceeded factors that encouraged adherence. This study showed that NICE guidance in isolation had little impact on GP prescribing. Where the guidance coincided with information from other sources, or personal experience, there was some evidence that technology appraisals triggered an increase in prescribing, but that this was not always sustained. The recommendations of NICE concerning zanamivir were universally rejected and there was evidence that this had undermined confidence in NICE recommendations in general.
