Q: Is CDAD more likely to occur with the use of broad spectrum antibiotics? A: Yes. Cephalosporins and clindamycin, which disrupt the GI flora, are related to a higher incidence of CDAD. As a consequence of the 4C1 conformation of the glucopyranose units, all secondary hydroxyl groups are situated on one of the two edges of the ring, whereas all the primary ones are placed on the other edge. The ring, in reality, is a cylinder, or better said a conical cylinder, which is frequently characterized as a doughnut or wreath-shaped truncated cone. The cavity is lined by the hydrogen atoms and the glycosidic oxygen bridges, respectively. The nonbonding electron pairs of the glycosidic-oxygen bridges are directed toward the inside of the cavity, producing a high electron density there and lending to it some Lewis-base character. The C-2-OH group of one glucopyranoside unit can form a hydrogen bond with the C-3-OH group of the adjacent glucopyranose unit. In the CD molecule, a complete secondary belt is formed by these H bonds, therefore, the CD is a rather rigid structure. This intramolecular H-bond formation is probably the explanation for the observation that CD has the lowest water solubility of all CDs. The H-bond belt is incomplete in the CD molecule, because one glucopyranose unit is in a distorted position. Consequently, instead of the six possible H bonds, only four can be established simultaneously. The CD is a non-coplanar, more flexible structure; therefore, it is the more soluble of the three CDs. Figure 5 shows a sketch of the characteristic structural features of CDs. United States of America --The Food and Drug Administration FDA ; has approved the first biotechnology product to treat patients with allergy-related asthma. Omalizumab Xolair ; , is a monoclonal antibody shown to be safe and effective for patients with moderate to severe allergy-related asthma inadequately controlled with inhaled corticosteroid treatments. In these patients, omalizumab has been shown to decrease the number of asthma exacerbations or episodes of airway narrowing that result in wheezing, breathlessness, and cough. The product is given as an injection under the skin. As a second-line treatment, it is recommended when first-line treatments have failed; it is not approved for children under the age of 12 and is only for patients with moderate to severe, allergy.
| Several adverse effects associated with tetracycline therapy19. It commonly produces gastrointestinal upsets e.g. vomiting, diarrhoea ; and vaginal candidiasis through changes in the mucocutaneous bacterial flora. Benign intracranial hypertension is a rare but important adverse effect of therapy with all tetracycline-group medications. Minocycline is more effective in the treatment of acne than tetracycline or doxycycline, but its widespread use as first-line treatment is precluded by the high cost. Minocycline can cause reversible vestibular disturbance e.g., dizziness, vertigo and ataxia ; . Minocycline causes a bluegrey discolouration of the skin, particularly in areas that are inflamed. Hepatitis and reactions resembling serum sickness and lupus have also been reported in association with use of the tetracyclines, particularly minocycline20. Erythromycin and co-trimoxazole offer alternative treatments. Although erythromycin and tetracycline are equally effective in the treatment of inflammatory acne21, erythromycin is chosen in practice less frequently because of the frequent emergence of resistant strains of P. acnes the presence of which is often associated with treatment failure ; 22. It also causes intolerable gastrointestinal side-effects in many patients. Co-trimoxazole effectively treats inflammatory acne; however, the potential for serious, though rare, side effects including hypersensitivity reactions e.g., toxic epidermal necrolysis ; and bone-marrow suppression generally limits its use to patients who have responded inadequately to the more commonly used oral antibiotics23. Although oral clindamycin improves inflammatory acne, its use in this setting has been virtually abandoned because of its association with pseudomembranous colitis24. Hormonal treatment improves acne by decreasing androgen-induced sebum production. Hormonal therapy may be indicated for women with characteristics that suggest a significant hormonal influence, i.e. inadequate response to other acne treatments, acne that begins or worsen in adulthood, premenstrual flares of acne, excessive facial oiliness, inflammatory acne limited to the area of male beard distribution and acne accompanied by mild to moderate hirsutism. However, long-term side effects have limited their use. Since the present line of treatment is associated with the above side effects this trial, which was conducted using alternative herbal medications showed promising results. CONCLUSION In this trial, we observed that in grade II acne, 56.25% of the patients had moderate response in the first week itself, 13.25% had good response and excellent response was obtained in 6.2% of the patients. In the second week, 37.50% had an excellent response and 56.25% had a good response. And at the end of 4th week, there was excellent response in 56.25% and 43.75% had good response. In third degree acne, there was only moderate response in the majority of patients during 1st week. In the second week, 78.33% had a good response. At the end of the third week, an excellent response was seen in 38.33% with a complete absence of lesions, and a total of 56.66% had a good response and ditropan.
Between 2% and 27% of the population in Western countries suffers from constipation with most studies suggesting a 12% to 19% prevalence.1 Most studies have found an increased prevalence of constipation in the elderly2 and in women. Lack of physical activity, low income, limited education, history of sexual abuse, and symptoms of depression are all risk factors for constipation.3 In the United States, constipation leads to approximately 2.5 million physician visits each year, nearly 85% of which result in laxative treatment.4 Laxative sales run in the several hundred million of dollars annually; nearly 60% of elderly people report regular laxative use. In a National Canadian Survey, 34% of individuals who reported constipation had seen a physician for their symptoms.5 Those patients who sought medical attention for their constipation scored lower on the mental and physical subscales of the SF-26 questionnaire compared to a control group without any functional gastrointestinal disorders.6 The traditional medical definition of constipation has been infrequent bowel movements, typically fewer than 3 per week, which is outside the range of normalcy for 95% of healthy adults in North America and the United Kingdom.7 In contrast, most patients define constipation by a variety of symptoms, including need for excessive straining, hard stools, unsuccessful defecation, abdominal discomfort, sense of incomplete bowel evacuation, and excessive time spent on the toilet.8 A consensus definition of constipation published in 1999 Rome II Criteria ; 9 and a recent American College of Gastroenterology Task Force10 definition of constipation incorporate many of these symptoms Table 1 ; . 1992. There was one important difference, however; the presently studied strain was more resistant to gatifloxacin and moxifloxacin--antibiotics not used earlier. Resistance to clindamycim was the same in both study groups. In the companion paper, Loo et al. performed a prospective study of C difficile infection in 12 hospitals in Quebec, identifying 1, 703 patients with a rate of 22.5 per 1, 000 hospital discharges, which is an increase, as well as higher 30-day mortality of 6.9%.24 The case controlled study showed a higher mortality with increasing age, risk factors of cephalosporin and fluoroquinolone use, and a dominant single strain--fluoroquinolone-resistant and producing the binary toxin. Why is this happening now? It is a combination of factors--perhaps a "perfect storm" for C difficile. First, is widespread use of quinolones. In an editorial, Gerding has proposed that the anaerobic activity of the newer quinolones disrupts the gut flora extensively and that acquired resistance by C difficile and proliferation of resistant clones led to the hospital outbreaks.28 Second, these strains may be more virulent--Warny et al. analyzed isolates from Quebec and noted in vitro marked increased production of toxins A and B 16- and 23fold, respectively ; , so they may cause more severe disease.29 The significance of the binary toxin is not known, but it resembles the iota toxin of C perfringens and may cause disease also. Third, the spread of C difficile will not be stopped by alcohol hand gels, now widely used but which do not inactivate spores. Fourth, stool diagnostic tests are imperfect and diagnosis may be missed or delayed, thus clinical suspicion is more important than ever. Fifth, hospital crowding, with limited private rooms and semiprivate rooms may make it difficult to prevent transmission. Finally, poor response to metronidazole and frequent recurrences could increase chances for transmission. What are the implications for us as clinicians? It has always seemed that some people became sick very quickly with severe PMC and toxic colitis that is not always due to a delayed diagnosis. Identification of theses strains supports this concept, making early recognition and treatment of C difficile disease now more important than ever. We must have a high index of suspicion in our hospitalized patients as well as older patients and even in otherwise healthy outpatients who develop symptoms on or after antibiotics ; and be aware of early clues such as fever, diarrhea, low serum albumin, and high white blood cell counts or leukemoid reactions. We must remember that some patients with toxic colitis may not have diarrhea. Abdominal distension must be carefully evaluated. In patients with severe colitis who are not improving on metronidazole, after 3 to 5 days, change to vancomycin will be important, in addition to early consultation with surgeons. As internists, we may not always recognize the appropriate time for surgical intervention as well as our surgical colleagues do. Hospital infections control units need to be vigilant and detect epidemics early. C difficile is widespread in hospital environments; the spores, which are resistant to antibiotics, can last for months to years before germinating and are not destroyed by alcohol hand gels. Hand washing with soap and water is crucial. Control of spread among patients and staff requires barrier methods including isolation, if feasible, and environmental cleaning with bleach. Antibiotic restriction policies may be needed to control epidemics. A Virginia teaching hospital with an epidemic of a clindamycinresistant strain was able to decrease CDAD rates by restricting the use of clindamycin.30 Similar restriction of quinolone antibiotics may be needed to control this strain.28.
