A. COLLECTION OF STOOL FOR STOOL CULTURE 1. Antacids, barium, bismuth, anti-diarrhea medication or oily laxatives mineral oil ; should not be used two weeks prior to collection of the specimen. 2. Collect the number of samples ordered by the physician. Do not collect more than one sample per day. 3. Collect the sample in a dry, wide-mouthed plastic container provided by the laboratory. Take care not to contaminate the sample with urine. A large plastic bag or "saran wrap" may be placed over the toilet seat opening and specimen passed into the bag. 4. Select an appropriate area of stool that may be bloody, slimy, or watery. Take a sample with the collection spoon or a wooden stick and put a thumb size sample of stool into the dry sterile container. 5. Specimens must be transported to the laboratory immediately. If the sample will be delayed more than two hours, refrigerate the sample. Samples should be delivered to the laboratory on the same day of collection. B. Take a sample of stool with the collection spoon provided in the caps of the containers. 5. Add stool to the containers until the liquid level is up to the "Fill to Here" line. If the stool is formed, material should be removed from the sides, ends and middle of the bolus. 6. Agitate each specimen with the spoon along the sides of the container, tighten the cap and shake firmly to mix specimen. 7. Label each specimen container with the patients name, MR# and the date time of collection. 8. Specimen should be transported to the laboratory immediately, but samples can be stored for 72 hours at room temperature. C. PATIENT PRECAUTIONS 1. Avoid contact of fixative solution with the skin and eyes. Should contact occur, flush with running water. If irritation develops, see a physician. 2. Fixative solutions are poisonous. If ingested, dilute by drinking milk or water, then call local poison center or physician immediately. 3. The role of pyrophosphate in removal is controversial, as our laboratory and others have shown WT RT removes AZTMP with a rate higher than that of AZTresistant RT at a physiological concentration of pyrophosphate, which argues against PPi-mediated removal as a mechanism of AZT resistance 14, 30 ; . Alternatively, AZT-resistant RT removes AZTMP more rapidly than WT RT at physiological concentration of ATP 30 ; . It has been established that ATP binds directly to RT at site proximate to the active site 33 ; with the -phosphate of ATP, acting as a nucleophile, directly attacking the phosphodiester linkage between AZTMP and the primer. The products of this nucleophilic attack are the primer free to incorporate dNTPs ; and the dinucleoside tetraphosphate 3 -azido-3 -deoxythymidine 5 ; tetraphospho 5 ; -adenosine AZTp4A ; . Together, these observations would suggest that ATP, and not PPi, acts to mediate AZTMP removal in the context of AZT resistance. We have thus chosen to study only ATP-mediated removal in this work, examining a host of Food and Drug Administration-approved compounds to determine whether a general mechanism of synergy exists. As shown in Fig. 1A, we show that the addition of 10 M nevirapine or efavirenz inhibits ATP-mediated removal of a wide panel of nucleoside analogs, including AZT, d4T, ddC, ; 3TC, ; 3TC to study an isomeric effect ; , and recently approved ; FTC. These results substantiate the broad ability of RT to remove chain-terminating analogs and that there exist interactions between the polymerase active site and nonnucleoside binding pocket 22 ; . In all cases, efairenz inhibited removal more effectively than nevirapine and accordingly parallels the potencies observed in the clinic. The relationship between NNRTI inhibition of nucleotide removal and stereoselectivity was examined by comparing primers chain-terminated with the oxathiolane stereoisomers, ; 3TCMP or ; 3TCMP. In each case, the effects of NNRTI inhibition on the rates of removal for ; 3TC were comparable with that of ; 3TC, showing that there is no stereochemical preference at the site of chain termination. Fig. 1B illustrates the clear inhibition of removal when either nevirapine or efzvirenz is added to the assay, in that both the rate and amplitude of AZTMP removal are decreased when an NNRTI is added. It is also interesting to note that in comparing removal by wild-type RT in the absence of NNRTIs, we see that "thymidine T ; " analogs are removed more effectively than the "deoxycytidine C ; " analogs compare time scale for d4TMP removal versus ddCMP removal, 3 and 6 h, respectively.
