INTRODUCTION There is a large body of literature describing potential health benefits of soy and soy products in humans and animal models. Soy phytoestrogens, which may play a major role in these effects, have been extensively investigated and reviewed [1-5]. The two major soy phytoestrogens, genistein and 25 daidzein, are isoflavones that have a structural similarity to 17estradiol and elicit or selectively modulate estrogenic responses by binding to both estrogen receptor ER ; and [6, 7], with higher affinity for ER [8]. Studies on soy isoflavones have focused primarily on genistein because it is a more potent estrogen than daidzein [9, 10], is typically present in higher quantities than daidzein and has other properties like inhibition of protein tyrosine kinases [11] and topoisomerase II [12, 13], and 30 induction of TGF [9]. Despite the limited estrogenicity of daidzein, some recent evidence suggests that its metabolite equol may play a critical role in phytoestrogen effects [2]. Daidzein is converted by microbial biotransformation in the intestine to the isoflavan equol, a biologically active metabolite [14]. Equol has a significantly longer half life in the circulation than genistein or daidzein [15], and binds to both 35 ER and , though like genistein and daidzein its binding affinity for ER is greater than that for ER [16]. Approximately 1 3 of humans produce equol; this may reflect differences in the intestinal microbial flora of this group compared with humans who do not produce equol [17, 18]. Setchell et al [2] have postulated that the beneficial responses from soy consumption may correlate with the equol producing status of individuals, suggesting a major role for equol as a biomarker for the effectiveness 40 of soy [2]. Further, rodents on commercial diets formulated with soy meal are continuously exposed to isoflavones [19] and have sustained high serum equol concentrations [20]. Such high equol levels may potentially have important implications in experiments measuring endpoints influenced by isoflavones [21]. Despite the potential importance of equol, there have been limited studies of equol effects in 45 vivo, due to the high cost of this compound and its limited availability. In addition, studies of equol effects in vivo [22, 23] have utilized equol injection, rather than the more physiological administration of equol in the diet, again due to the high cost of equol precluding dietary studies. Furthermore, due to difficulties in quantitating equol in biological fluids [2], there have been no studies correlating various serum equol concentrations with estrogenic or other biological effects. 50 Development of a technique for producing high quantities of a racemic mixture of equol [16] has provided a unique opportunity to more fully characterize the effects of equol in vivo on a variety of!

Addition of genistein activated Cl currents 10-fold, and the Po of G551D-CFTR increased to 0.49 0.12, which is a Po similar to wild-type CFTR. In cystic fibrosis CF ; epithelial cells homozygous for the traffickingimpaired F508 mutation, forskolin and genistein activated Cl currents only after 4-phenylbutyrate treatment. These data suggested that genistein activated CFTR mutants that were present in the cell membrane. Therefore, we tested the effects of genistein in CF patients with the G551D mutation in nasal potential difference PD ; measurements in vivo. Transitional cell carcinoma is the most important cancer of the urinary tract and occurs in the renal pelvis, ureter, urinary bladder, and urethra. The incidence of this cancer and the number of deaths from this cancer are generally increasing, notably in economically developed societies. It is well known that field cancerization is an important characteristic of transitional cell carcinoma and may affect the natural course of disease progression. For example, enhanced expression of tumor-associated antigens could be shown in normal-appearing urothelium 13 ; , and the degree of epithelial dysplasia predicts the risk of disease progression 4 ; . As result, it is imperative to modulate carcinogenesis as early as from the initiated cells in the field mucosa. A number of epidemiological studies have reported that increased soy consumption is associated with a reduced risk of breast, colon, and prostate cancer for peoples living in Asia as compared with peoples living in America and Western Europe 5 ; . Further support for the potential of isoflavones as natural chemopreventives is the dose-dependent response of urinary isoflavone excretion to soy consumption from low to higher doses 6, 7 ; . Animal studies also found that soybean feeding has a protective effect on bladder carcinogenesis in Swiss albino mice 8 ; and on a transplantable murine tumor 9 ; . A recent report showed that genistein 5, 7, 4 -trihydroxyisoflavone ; , daidzein 7, 4 -dihydroxyisoflavone ; , biochanin-A, and phytochemical concentrate tend to cause a dose-dependent inhibition of proliferation in J82 human cancer cells 9 ; . A G2-M-phase cell cycle arrest could be demonstrated in both the J82 and UM-UC-3 cell lines. Because isoflavones are excreted in human urine, and the levels are positively associated with the frequency of soy intake 10 ; , this group of compounds deserves investigation in the context of dietary chemopreventives for urinary tract cancer. This study was performed to address the following questions: a ; do the cancer protective effects apply to bladder cancer of various histological grades? b ; is there any toxicity to normal urothelium if isoflavones are used as treatment? and c ; what is the biological efficacy of mixed soy isoflavones at concentrations found in the range of human urine excretion?.
