Topamax
The good news is that the harm caused by toxic chemicals is preventable. Replacing toxic chemicals with safer alternatives can reduce the number of people who develop cancer, learning disabilities, and reproductive and neurodegenerative disorders.
Topamax dosage for adults: topamax therapy usually begins with a dose of 50 milligrams once daily during the first week.

Roussel Baker Norton ; , and class I and III antiarrhythmic agents. The primary outcome measure was the change from baseline in the frequency of seizures. The authors expressed the change as the "R-ratio." Secondary outcome measures were the responder rate, the percentage of patients free of seizures, and the median percentage change in seizure frequency. Of 288 patients randomly selected for treatment, one patient did not take at least one dose of the study medication. Approximately 50% of the patients enrolled took two concurrent antiepileptic drugs, and almost 30% took three concurrent antiseizure medications. Carbamazepine Tegretrol, Novartis ; , lamotrigine Lamictal, GlaxoSmithKline ; , and topiramate Topamax, Or thoMcNeil ; were the three most commonly used anticonvulsant drugs. Others used in the study included phenytoin Dilantin, Pfizer ; , phenobarbital, valproate e.g., Depakene, Abbott ; , and clobazam Frisium, Hoechst ; . Both treatment arms displayed a statistically significant reduction in the frequency of seizures, compared with placebo. The reduction in seizure frequency obser ved during the study was doserelated: with 150 mg day, the R-ratio was 11.5; with 600 mg day, it was 31.4; and with placebo, it was 0.9. Patients receiving pregabalin 150 mg day and 600 mg day also experienced a reduction in seizure frequency from baseline measures by 20.6% and 47.8%, respectively ; , whereas patients receiving placebo displayed an increase in seizure frequency by 0.9% ; . A significantly greater percentage of patients receiving pregabalin 600 mg day 43.5% ; were considered to be treatment responders noted by a reduction of 50% or more in seizure frequency ; , compared with patients receiving pregabalin 150 mg day and placebo P .0001 ; . Median reductions in seizure frequency were 16.5% with pregabalin 150 mg day and 42.6% with 600 mg day. With placebo, seizure frequency increased by 1.3%. The ADEs in this study were similar to those obser ved in the Dworkin and Rosenstock studies.2, 6 The percentage of patients who withdrew from the study because of ADEs increased with the higher dose of pregabalin. Furthermore, the number of concurrent antiepileptic.

Patients with Hepatic Disease In hepatically impaired patients, topiramate plasma concentrations are increased approximately 30%. This moderate increase is not considered to warrant adjustment of the TOPAMAX dosing regimen. Initiate topiramate therapy with the same dose and regimen as for patients with normal hepatic function. The dose titration in these patients should be guided by clinical outcome, i.e. seizure control, and avoidance of adverse effects. Such patients will require a longer time to reach steady-state at each dose. Geriatrics See WARNINGS AND PRECAUTIONS section. Missed Dose The missed dose should be taken as soon as possible. If it is almost time for the next dose, the missed dose should not be taken. Instead, the next scheduled dose should be taken. Doses should not be doubled. Administration Tablets should not be broken. TOPAMAX Sprinkle Capsules may be swallowed whole or may be administered by carefully opening the capsule and sprinkling the entire contents on a small amount teaspoon ; of soft food. This drug food mixture should be swallowed immediately and not chewed. It should not be stored for future use. The sprinkle formulation is provided for those patients who cannot swallow tablets, e.g. pediatric and the elderly and topiramate.
Preschoolers Continued from page 20 When you assess a young child with problem behaviors, you wonder, "What kind of person is he going to be?" There is never a simple answer. Hyperactivity, anxiety, aggression, or depression can be from many sources and can have many outcomes. Some of the worst-behaved young children grow up to be fine adults. There are no longitudinal studies to show that bipolar-diagnosed children become bipolar adults. What worries me most is that we will miss issues that need immediate action, such as abuse, or treatable issues of parenting, while diagnosing a three year old bipolar and giving them a mood-stabilizer. Feeding HIV-infected mothers mix fed their infants foods such as honey, water or sugar water during their postpartum stay in the hospital. This contrasts with the 11% n 4 ; of the mothers who top fed and also gave breast-milk 2 11, P 0.001 ; . We found two distinct types of mixed feeding in this population: 1 ; the giving of some liquids and foods to the child during the first 3 d after birth only, and 2 ; the giving of mixed feeds to the child regularly for 3 d. In our sample, 28 74% ; of mothers were classified as mixed feeders for 3 d only, with the remaining 10 26% ; mixed fed on a more regular basis. Mixed feeding that occurred during the first 3 d after delivery usually occurred on the advice of doctors and relatives and was related to the mother's inability to produce sufficient milk for the child, c-section discomfort and problems of feeding top milk during the night. Women who mix fed 3 d fed their child using only one type of milk either breast-milk or top milk ; but in addition regularly gave locally available gripe water6 and herbal preparations thought to promote infant growth see quotes below ; . "I started breast-feeding because my husband has lost his job. Even the doctor told me to breast-feed for 4 mo. I give plain water, sugar water and herbal preparation, along with breast-milk." "My sister-in-law forced me to breast-feed to my baby. So under her pressure I breast-fed my baby for 3 d, 3 times-every day for 15 min. Nobody at our home knows about my HIV status." "My child used to cry during night, so my husband suggested to breast-feed during the night and top feed during the day." Subsample study. Of the 101 women originally interviewed, 41 HIV women were eligible and consented to additional interviews. Of the 41 HIV women, 1 child was stillborn and 1 was a home delivery, leaving 39 women who completed the antenatal, delivery and in hospital postpartum visits. Of this sample, a greater proportion of women decided to breast-feed 56%, n 22, ; as opposed to top feed 36%, n 14 ; . However, other demographic characteristics of this sample were similar to those of the larger sample. Infant feeding intention was most influenced by the counselor n 21, 54% 31 28% ; relied on themselves and 6 15% ; on the doctor's advice. Most women disclosed their HIV status to their husbands n 27, 69% ; . However, regardless of whether they told their spouse, they were less likely to discuss their results with their in-laws or parents n 12, 31% ; . Top feeding mothers were significantly more likely to disclose their HIV status to their husbands and relatives than breast-feeding mothers [n 12 85% ; top feeding compared with n 12 54% ; breast-feeding mothers P 0.05 ; ]. For those 12 31% ; women who did not tell their spouse, six 50% ; did so out of fear. In-laws were less frequently told, either out of concern for them n 9, 33% ; or because of fear of blame n 8, 30% ; . Parents, on the other hand, were not told so as prevent any tension or pain to them. If the women revealed their HIV status to a family member, they were very likely to seek their advice on feeding methods. However, the most influential individual in helping them make the final feeding choice multiple responses allowed ; was the counselor n 29, 74% ; , followed by themselves n 21, 54% ; and their husbands n 10, 26% ; . After delivery, it was mainly the counselor who spoke with the women about infant feeding practice n 21, 54% 11 women 28% ; received no guidance after delivery, 6 15% ; spoke with husbands and 5 13% ; spoke with a doctor. All of the breast-feeding women knew of the risks of HIV.

People with panic disorder perceive their own physical and emotional well-being as poor. They use alternative therapies and seek medical help more often than do those in the general population. Any causal connection between anxiety and medical disorders is unclear. Effects on the Heart. Anxiety has been associated with a number of heart problems, although the mechanisms are unclear. Some studies have observed a tendency for unhealthy cholesterol levels in people with panic disorder. A 2001 study reported an association between thicker blood vessels in both women and men with anxiety and with hardening of the arteries in men but not in women ; .Another study indicated that people who experience anxiety are more likely to develop high blood pressure than are those who are not anxious. Both anxiety and depression have been associated with a poor response to treatment in heart patients, including a worse outcome after heart surgery. Some researchers speculate that intense anxiety might trigger abnormal and dangerous heart rhythms in people with existing heart problems. In other studies, panic disorders, post-traumatic stress disorder, and phobias have been associated with a higher rate of sudden death from cardiac events, including heart attack. Effects on the Gastrointestinal Tract. Anxiety frequently accompanies gastrointestinal conditions. Of note, half the cases of irritable bowel syndrome are associated with anxiety. Effects on Headache. One study reported that 32% of people with chronic tension headaches met criteria for anxiety. It isn't clear whether the psychological disorder preceded or followed the onset of headaches. Similarly, another study reported that young girls with anxiety disorders were three times more likely to have chronic headaches than those without the disorder. Headaches in both studies were also strongly associated with depression and zantac.

Consumer segment sales in 2002 were $6.6 billion, an increase of 3.9% over 2001. Of the 3.9% increase in Consumer segment sales over prior year, 4.6% was operational growth with currency negatively impacting sales growth by 0.7%. Domestic sales increased by 4.5% while international sales gains in local currency of 4.6% were offset by a negative currency impact of 1.5%, resulting in total international growth of 3.1%. Consumer sales achieved strong growth in skin care products NEUTROGENA, CLEAN & CLEAR and AVEENO ; and BAND-AID wound care products, as well as in McNeil Nutritionals' SPLENDA sweetener products and VIACTIV calcium chews. Consumer segment sales in 2001 were $6.3 billion, an increase of 0.8% over 2000. Domestic sales increased by 1.4% while international sales gains in local currency of 6.8% were offset by a negative currency impact of 6.7%, resulting in total growth of 0.1%. Consumer segment sales in 2000 were $6.3 billion, an increase of 0.4% over 1999. Domestic sales increased by 2.8% while international sales gains in local currency of 4.3% were offset by a negative currency impact of 6.6%, resulting in a total decrease of 2.3%. Pharmaceutical segment sales in 2002 were $17.2 billion, an increase of 15.5% over 2001 including 16.4% growth in domestic sales and 13.5% total growth in international sales that includes a 2.4% positive effect of currency. Of the 15.5% increase in Pharmaceutical segment sales over prior year, 14.8% was due to operational increases, with currency positively impacting sales growth by 0.7%. Sales growth reflects the strong performance of PROCRIT EPREX, for treatment of anemia; REMICADE, a treatment for rheumatoid arthritis and Crohn's disease; RISPERDAL, an antipsychotic medication; DURAGESIC, a transdermal patch for chronic pain, and TOPAMAX, an anti-epileptic medication. Sales of PROCRIT EPREX accounted for 11.8% of total Company revenues for 2002 and 10.6% in 2001. Johnson & Johnson markets over 100 prescription drugs around the world, with 30.5% of the sales generated outside the United States. Thirty-three drugs sold by the Company had 2002 sales in excess of $50 million, with 24 in excess of $100 million. The rate of growth for sales of PROCRIT and EPREX was slowed in the latter half of 2002 as a result of new competition for PROCRIT. Sales growth may also have been affected by rare reports of Pure Red Cell Aplasia PRCA ; in chronic renal failure CRF ; patients administered EPREX subcutaneously. The Company's on-going investigation of PRCA in CRF patients indicates that the occurrence of PRCA continues to be rare. During the second quarter of 2002, the Company completed its acquisition of Tibotec-Virco N.V. for approximately $320 million. Tibotec-Virco N.V. is a privately-held biopharmaceutical company focused on developing anti-viral treatments, with several promising compounds in development for the treatment of infectious diseases including HIV. During the fourth quarter of 2002, the Company received U.S. Food and Drug Administration FDA ; approval for LEVAQUIN levofloxacin ; for an additional indication for the treatment of nosocomial pneumonia, the second most common hospital-acquired infection. The Company also filed several new drug applications with the FDA. These include TOPAMAX topiramate ; for the prevention of migraine headaches in adults as well as for use as a monotherapy treatment in epilepsy it is currently approved as adjunctive treatment ; , LEVAQUIN for a five-day treatment of community-acquired pneumonia, and RISPERDAL risperidone ; as both adjunctive and monotherapy treatments of bipolar disorder. Also in the fourth quarter of 2002, the Company announced a definitive agreement to acquire OraPharma, Inc., a specialty pharmaceutical company focused on the development and commercialization of unique therapeutics in oral health care products. The acquisition will provide entry into the oral health professional marketplace by providing a synergistic line of prevention and treatment products to maintain periodontal health. The transaction is valued at approximately $85 million, net of cash, and closed in the first quarter of 2003. Pharmaceutical segment sales in 2001 were $14.9 billion, a total increase of 17.3% over 2000 including 21.3% growth in domestic sales. Operationally, international sales increased 14.2% but were partially offset by a negative currency impact of 4.9%, resulting in total growth of 9.3%. Pharmaceutical segment sales in 2000 were $12.7 billion, an increase of 12.7% over 1999 including 21.4% growth in domestic sales. Operationally, international sales increased 7.6% but were more than offset by a negative currency impact of 8.9% resulting in a total decrease in sales of 1.3%. Sales growth was partially offset by the restricted access of PROPULSID in a number of markets around the world. Worldwide sales in 2002 of $12.6 billion in the Medical Devices & Diagnostics segment represented an increase of 12.9% over 2001. As currency had no impact on sales growth, the 12.9% total increase is also the operational sales increase over prior year. Domestic sales were up 13.0% and international sales increased 12.8% over the prior year. Strong sales growth was achieved in each of the major franchises within this segment: Cordis' circulatory disease management products; DePuy's orthopaedic joint reconstruction and spinal products; Ethicon's wound care, surgical sports medicine and women's health products; LifeScan's blood glucose monitoring products; Ethicon Endo-Surgery's minimally invasive surgical products; Ortho-Clinical Diagnostics' professional diagnostic products and Vistakon's disposable contact lenses. 30.

P81 Evaluation of Bacterial Contamination in Water Bottles Maintained for Two Weeks for Individually Housed Mice CJ Haist * , JM Cadillac, RC Dysko Unit for Laboratory Animal Medicine, University of Michigan, Ann Arbor, MI Standard husbandry practice at our institution requires that water bottles in mouse cages be changed at least once per week, regardless of the cage population or ventilation status. Several studies at our institution require that mice be housed individually, and our animal care staff realized that at seven days time of bottle change ; an 8ounce bottle provided to a singly housed mouse still contained approximately 5 ounces of water. This prompted us to evaluate whether an 8 ounce bottle could be provided to singly housed mice for two weeks, thus reducing the labor needed to maintain the cage. Mice were singly housed in either nonventilated static ; or ventilated microisolation racks and provided with filtered tap water in an 8 ounce glass bottle static boxes ; or 9 ounce polycarbonate bottle ventilated boxes ; . We provided fresh water to mice on day 0. Water samples were taken on days 0, 7, 10, and 14. Samples 3cc ; were obtained from the water bottle and filtered 0.45 m pore size ; into a sterile container. The filters were placed on blood agar plates and incubated at 37C for five days. Mice were from one of two strains: C57BL 6 or a hybrid strain made from a cross between CB6F1 female ; and C3D2F1 male ; . Mice were housed in either one of two buildings, and one of two types of caging ventilated versus nonventilated ; for a total of eight groups. One group hybrid mice in ventilated racks in Building #1 ; had contaminants on day 7 1 3 ; , and 14 3 ; Bacteria identified included species of Micrococcus, Staphylococcus, Streptococcus, and Pantoea formerly Enterobacter agglomerans ; . The other seven groups had infrequent positive cultures, typically yielding very few organisms. Of the 84 samples, bacteria were cultured from only 7 8.3% ; . The bacteria in most cases were airborne contaminants. None of the boxes from any of these seven groups had more than one positive sample. We were unable to discern why the hybrid mice in ventilated racks housed in building #1 had more significant bacterial contamination, as the other groups served as controls for strain, cage type, and building. We concluded that it would be safe to adopt the practice of changing water bottles once every two weeks for singly housed mice. P82 The Effects of Cage Density on Reproductive Performance and Environmental Conditions in Conventionally Housed FVB N Mice Utilized for the Production of Transgenics TS Golding * , YY McNeil, CL Kimbrough, R Rahn GlaxoSmithKline, Research Triangle Park, NC Facilities creating transgenic mice often utilize intensive mating schemes to rapidly maximize production. Routinely, subjective assessments of cage sanitation in combination with the objective measurements of ammonia production have been utilized to gauge the appropriateness of such housing conditions. To determine if performance-based criteria related to mouse reproduction supported these commonly used assessment criteria, a study was designed to examine the effects of high density breeding on several reproductive parameters. Subtle changes in reproductive performance were measured with sample sizes set to achieve an 80% power based on historical reproductive data from this mouse strain. Accordingly, age-matched primiparous FVB N mice in 120 cages, maintained in either paired or trio matings were monitored in three separate cohorts over a period of 3 months. Cage changing was set to once every 7 days; this permitted detection of ammonia levels that exceeded the often-used human threshold limit value of 25 parts per million ppm ; . Environmental.

Kiss chapped lips goodbye Give your lips extra care and attention this winter season too. "Lips are more sensitive than the rest of our body, and they are certainly exposed to the same harsh environment, " says Dr. Lopez. Because lips have fewer sebaceous oil ; glands and no melanin pigment, they are more prone to dryness and sun damage. "Dry and chapped lips not only feel irritated, but they also can appear more wrinkled, " she says. Kiss dry and chapped lips goodbye with these tips: Apply lip moisturizer or lip balm daily to keep lips healthy and to maintain their smooth feel and appearance. Look for products that contain petrolatum, dimethicone, aloe vera, cocoa butter, vitamins A and E, and beeswax, which are some examples of moisturizers and skin protectants. Add an extra layer of lip protection and clomid.

Within two months of starting therapy, the man's CD4 + count rose to 305 cells. Five months after he began therapy, his viral load was below the 500 copy mark. A year after he sought medical attention, CAT-scans of his body revealed that his KS lesions had disappeared. As well, anti-HHV8 antibodies could no longer be detected, suggesting that this virus had been contained.

| Topamax to lose weightMechanism of action is probably genotoxic. The seven remaining compounds marked superscript 4 in Table II ; are negative in GSA with no carcinogenicity data, and have negative or sporadic positive genotoxicity data. To determine how representative this subgroup of pharmaceuticals was, a simple comparison was made between the numbers of each regulatory test performed within the group of 51 and the Snyder collection as a whole Table III ; . This subgroup has a slightly higher proportion of compounds with at least one positive result from the standard battery 43.1% compared with 28.7% in the larger set of 352 compounds ; , and this is mainly accounted for by a higher proportion of Ames positive and in vitro cytogenetics positive compounds. This is however a much lower proportion than that in the data from 102 compounds previously published in the validation study 9 ; in which 80% had positive data from at least one of the standard battery of tests. This is important as the prevalence of positive compounds in a test data set has a profound impact on the usefulness of the calculated `predictivity' of an assay, especially where compound data sets contain a disproportionally high number of compounds with positive data 15 ; . A discussion of each compound and an overall assessment of their genotoxicity and carcinogenicity where data is available ; is given in Appendix I. Study of 2698 J&J compounds J&J put 2698 potential drug candidates through the GSA during pre-regulatory screening. 2351 have also been tested with Ames II. Of the 2351, 164 7% ; were positive in Ames II, with and or without S9 metabolic activation and 176 7.5% ; were positive in the GSA. Twelve 7% ; of the 176 GSA positives were positive in Ames II. This underlines the fact that the Ames II and GSA assays each detect a different but overlapping spectrum of genotoxins, reflecting both the differences between prokaryotic and eukaryotic test organisms and their different endpoints mutation and DNA damage-induced transcription respectively ; . As part of the company's policy in compound selection, compounds with Ames II positive data were rejected. The archived GreenScreen data was referenced when later regulatory tests on the Ames II negative compounds gave positive regulatory genotoxicity data. Ten such compounds emerged that gave positive results in the mouse lymphoma assay and or the in vivo micronucleus assay. Of these, six were positive in GSA. No in vivo micronucleus assay negatives were positive in GSA. These results underline how the GSA can be used to help prevent important genotoxic drug candidates from persisting in development using valuable resources that could be used to develop compounds more likely to be successful drug candidates.
