The presenter was noted that the study was halted by J&J, who is working on an improved formulation to enhanced tolerability and simplify dosing. Data shared: o At week 44, the mean percent change in weight for the two Topamax arms were 15% and 17% reductions, compared to a 9% reduction for those on placebo there has been a runin during which everyone was on a low-calorie diet-induced weight loss regimen in which an 8% weight loss was required in order to continue ; . o Adverse events: 13% dropped out in the Topamax arms compared to 8% in the placebo arms. In discussions with doctors, I have heard discussion of nausea, reduced activity levels, ability to function, etc. I understand that some populations, particularly those younger patients with no cognitive problems, appear to tolerate the drug well, though others have had major problems with the current formulation, which, again is not the formulation that J&J ultimately intends for obesity use. Anecdotally, to date, I have heard more caution on this drug than anything clearly, it will be interesting to see results with new formulation in large populations over a longer time period. This drug has only a minimal affinity for muscarinic acetylcholine receptors and therefore causes less dry mouth, constipation, or other such effects than do the tcas. The thin-section CT scans obtained in 60 patients with drug-induced lung disease were retrospectively reviewed. These 60 patients represented all patients who were identified from the clinical records and CT data files at our institution between January 1993 and December 2000. Diagnosis of drug-induced lung disease was established by means of the following criteria 6, 7 ; : a ; development of new symptoms and radiologic abnormalities while the patient was being treated with the drug, b ; absence of congestive heart failure, c ; absence of marked occupational exposure, d ; absence of an alternative explanation for the pulmonary abnormalities observed, and e ; resolution of the pulmonary abnormalities after cessation of treatment with the drug suspected of causing the abnormalities. When there were, among associated drugs, substances known to induce lung damage, the resolution of pneumonitis was observed after withdrawal of treatment with the drug implicated, despite continuation of treatment with the associated drugs. The patients who fulfilled all previously mentioned criteria were included in this study. Patients who were thought to have acute respiratory distress syndrome caused by the drug, who did not have resolution of abnormalities after cessation of treatment with the drug, and who died were excluded from the study according to the previously mentioned criteria. In these patients, the cases included pneumonitis induced with gemcitabine Gemzar; Eli Lilly, Japan ; in two cases, that induced with paclitaxel Taxol; Bristol Myers Squibb, USA ; in one case, and that induced with 7ethyl-10- 4-[1-piperidino]-1-piperidino ; carbonyloxycamptothecin, CPT-11 irinotecan hydrochloride; Topotecin, Daiichi, Japan ; , in one case. Sixty patients who met our criteria were included. Informed consent was provided by the patients, and the study was approved by the hospital's internal review board. The patients were referred for CT because of unexplained symptoms and pulmonary abnormalities while they were receiving the medications. The duration of use of the medication prior to CT examination ranged from 2 days to 2 years median, 1 month ; . Examination of sputum in all patients and of bronchoalveolar lavage fluid in 28 patients disclosed no viral, fungal, bacterial, or parasitic pathogens. The external laryngeal nerve may also be injured and this results in a weakness in the voice although the sound of the voice is unchanged. Difficulty may be found in reaching the high notes when singing, the voice may tire more easily and the power of the shout reduced. Careful surgery reduces the risk of permanent accidental injury to a very low level but cannot absolutely eliminate it. Injury to both recurrent laryngeal nerves is extremely rare but is a serious problem and may require a tracheostomy tube placed through the neck into the windpipe ; . Other hazards: Most parathyroid operations are straightforward and associated with few problems. However all operations carry risks which include postoperative infections e.g. in the wound or chest ; , bleeding in the wound and miscellaneous problems due to the anaesthesia, but these are very rare. Bleeding in the wound can be a serious problem if it occurs but the chance of a significant bleed needing you to return to the operating theatre within a day or two after your operation to clear out the blood is small in the region of 1`in 50 ; . Results of surgery Operations to correct RHPT are intended to remove or reduce symptoms which you are having which may be attributable to the RHPT. You should discuss with your surgeon or renal physician the reasons why you have been referred for surgery and their expectations of improvement. Parathyroidectomy may be effective at controlling the RPHT and reducing future damage but may not necessarily improve your current symptoms or restore damaged bones, nor can it be expected to alter your need for renal replacement therapies such as dialysis. In most people the stimulus to parathyroid overactivity remains even after successful surgery, but there is always a risk that parathyroid tissue which has been left in your body may enlarge and lead to recurrence of your symptoms. About one patient in ten undergoing parathyroidectomy for RPHT will require a further operation on the parathyroid glands in the future. In other patients parathyroidectomy may remove all functioning parathyroid tissue and you may require continuing medical treatment to sustain a normal blood calcium level. It is not possible to predict in advance what the long term result of parthyroidectomy will be on your blood calcium levels, but in the majority of patients the operation will be beneficial.
